What do I need to do if I’m a new patient?

On the menu, if you click on the Forms tab, you can complete the set of new patient forms before arriving to your appointment. Make sure you bring medication list and any other information the provider needs to know.

What insurance do you accept?

We accept most insurances. If you are unsure feel free to give us a call and we will find out for you.

What are your office hours?

Monday: 9 a.m. to 7 p.m.

Tuesday: 9 a.m. to 6 p.m.

Wedneday: 9 a.m. to 7 p.m.

Thursday: 9 a.m. to 6 p.m.

Friday: 9 a.m. to 4 p.m.

Do you provide phone consulations?

Unfortunately, we do not provide phone consultations. We recommend that you schedule an appointment with a provider and they will be able to assist you.

How do I find out my lab results?

Typically our staff with contact you if there are any lab abnormalities. However, please call our office on status of lab results.

How do I get a refill?

If you have not been seen in the past 3 months you must schedule an appointment before you can get your refill. Otherwise, please call your pharmacy and let them know which prescription needs to be filled. We will not refill prescriptions over the weekend.

How to I access my medical records from Complete Medical & Skin Care?

On the menu, if you click on the Forms tab there is a form called Authorization to Release Records to self from Complete Medical & Skin Care. You must clearly complete that form and you can either mail it or drop it off at our office.

Are cosmetic procedures covered by my insurance?

Unfortunately, cosmetics are self-pay and are not covered by insurance.

How can I transfer my records to another practice?

On the menu, if you click on the Forms tab there is a form called Authorization to Release Records from another doctor’s office. Please complete that form and we will contact that office for futher information.

What should I do to cancel an appointment?

Call within 24 hours prior to your appointment to cancel or else you will be subject to a $35 no show fee.
